Family Practice/Primary Care Nurse Practitioner

We are seeking a dedicated and experienced Nurse Practitioner (NP) to join a collaborative clinical care team providing high-quality, evidence-based care in a primary care setting. Position Summary As a MinuteClinic Nurse Practitioner, you will work alongside a supportive clinical care team to deliver comprehensive, patient-centered care. You will be responsible for assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions while promoting preventive health and wellness. Key Responsibilities Provide evidence-based primary and family care , including assessment, diagnosis, treatment, and health counseling Develop and manage comprehensive care plans for patients Perform routine exams and diagnostic testing to support clinical decisions Educate patients on healthy lifestyle choices and disease prevention Support chronic condition management and preventive screenings Coordinate care with other healthcare professionals Encourage patient engagement in their own health and wellness Administrative Duties Include: Opening/closing clinic operations Inventory management and maintaining clinical supplies Patient follow-ups and appointment management Insurance verification and payment collection Ensuring a clean, safe, and organized clinical environment Requirements Master s Degree from an accredited Family Nurse Practitioner program Current National Board Certification and active state NP license Minimum 1 year of recent Primary Care experience (Family Medicine, Primary Care, or Urgent Care) Strong understanding of chronic disease management and quality metrics Ability to work independently and autonomously Excellent communication, organization, and multitasking skills Comfortable with technology and electronic medical systems Willingness to obtain multi-state licensure Ability to work in a remote collaborative care team environment Schedule & Work Environment hours per week Rotating schedule including every 3rd weekend Float position across locations Individual start Why Join Us?
