OT - Occupational Therapist

Physical Therapist / Occupational Therapist / Speech-Language Pathologist

This is a 13-week contract position.

Monday-Friday, 5 × 8 hour shifts, 40 hours per week.

Your primary responsibility will be to provide in-home health care as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. You will deliver evaluation, treatment plans, and interventions, ensuring quality care for patients.

Conduct evaluations and develop individualized treatment plans.

Implement therapeutic interventions and monitor patient progress.

Document patient care in Homecare Homebase (HCHB) accurately.

Participate in weekly case conferences.

May cover Shasta Lake south to Orland, and assist in other areas as needed within a 60-mile radius.

Manage a caseload of 4-6 visits per day, depending on the type of visit.

Achieve weekly productivity of 28-32 points.

Experience in Home Health and OASIS documentation is required.

Experience with Homecare Homebase documentation is required; exception may be made for candidates willing to undergo training for the first two weeks at a reduced rate.

Valid state licensure as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
