Reema Health is seeking a Community Health Navigator (Community Guide) in the Edmonds, WA area to build relationships with members, guide them toward engagement in their own care, and help address social barriers using Reema's technology platform. The role requires Monday–Friday availability, 25–50% travel in a 30‑mile area, remote/field based work, and duties include outreach, caseload management (80‑120 members), coordinating referrals, and documenting care plans while maintaining HIPAA. #J-18808-Ljbffr Reema Health
