Responsibilities
- Assess patients to identify movement dysfunction and review medical history and referrals
- Develop and implement individualized rehabilitation plans to improve function after injury or surgery
- Guide and coach patients through therapeutic exercises and stretches; use hands-on techniques as appropriate
- Document patient conditions, care plans, therapies, and progress; maintain accurate records
- Communicate progress and clinical priorities with physicians, nurses, and the patient care team; engage with family as needed
- Collaborate with other clinicians to optimize outcomes; participate in team-based care and discharge planning
- Maintain patient confidentiality and provide culturally sensitive care across developmental and sociocultural backgrounds
- Provide education to patients and families to reduce anxiety and support ongoing care

Requirements
- Education: Master’s Degree from an accredited physical therapy program
- Licenses/Certifications: Current Physical Therapist state license; Current Basic Life Support (BLS) or ability to obtain within 6 months of hire
- Experience: Experience working directly with patients in a long-term care or hospital setting
- Skills/Abilities: Strong documentation, excellent verbal communication, teamwork, and patient-centered care
- Ability to work in a mixed outpatient/inpatient setting (approximately 90% outpatient, 10% inpatient)
