Certified Registered Nurse PractitionerA Certified Registered Nurse Practitioner (CRNP) provides primary care services, including assessing and managing patients in the clinic, through the patient centered medical home concept, following the established standards and practices of the clinic. The position combines the roles of provider, mentor, educator, researcher and administrator. The CRNP must honor the Commonwealth of Pennsylvania's Nurse Practitioner's guidelines and ethical requirements. The CRNP must have and exercise the ability to communicate effectively, both verbally and in writing, with advanced interpersonal communication skills, and must at all times comply with applicable standards and exercise appropriate care to protect confidentiality, privacy and security of patient protected health information. The CRNP must also be willing to travel to other clinics to provide care and coverage as needed.The position reports to the Supervising Physician for medical issues, to the Practice Manager and Co-Directors of CRNP & PA Services for practices and policies and Medical Director of the clinic. No staff report to this position.While living and demonstrating our Core Values, the Certified Registered Nurse Practitioner shall perform the following essential job duties and functions:General ManagementObtain, compile and record patient medical data, including health history, progress notes and results of physical examinationInterpret and integrate data to determine appropriate treatment plans as neededInstruct and counsel patients. Communicate clearly and effectively with patients and other health-care workersCollaborate with physicians in managing acute and long-term medical needs of patientsOrder necessary diagnostic tests, such as x-ray, electrocardiogram, and laboratory testsInterpret diagnostic test results including to assess and identify deviations from normalDiagnose and treat acute, chronic, and long-term healthcare issues. Interview and advise patients regarding health and illness preventionRecommend resources to meet patient and family needsProvide counseling and instruction regarding patient treatment and follow-up careProvide effective and meaningful patient educationAssist in supervision of Medical Assistants and Licensed Practical NursesEffectively identify, evaluate, and address disease prevention and health promotion issues of the population in the practice while administering quality patient careUnderstand and participate in meaningful use dataUtilize and document in the electronic health recordAdminister emergency care per BCLS, ACLS or PALS standardsDocument medical records with all patient encounters in the office and on the telephone while staying compliant and up-to-date with the EMR processUnderstand and adhere to "incident-to" and "non-incident-to" service and billing guidelines using proper procedures based on service conditions at the timeExercise HIPAA confidentiality and security measures at all times during office hours and outside of the officeDemonstrate responsibility for self-learning through participation in continuing education activities and conferencesParticipate in quality improvement reviews and activities in the clinical settingServe as a clinical resource for staff, clients and families which includes answering triages, help streamline patient flow in officePractice as an outpatient provider with thirty-two (32) hours of patient care per week (with availability for 3 patient slots per hour, 96 patient slots per week) and eight (8) hours of administrative time per week, and must be present on-site at least forty (40) hours per week, which includes administrative time. Administrative time must be coordinated and planned to occur outside of patient care hours and when there is otherwise an adequate number of providers to cover overall patient care responsibilities. The particular days scheduled will be worked out with the Medical Director and Practice ManagerMust have zero charts overdue > 10 days no more than 100 at end of monthMonitor lab and document box of other providers as assigned and direct any questions or concerns to the Medical DirectorWork some late nights, on average one to two nights per weekParticipate in the outpatient call schedule, which typically includes one weekend per month and the occasional weekday as needed, with physician as backupParticipate in weekend rotation including up to two Sundays per month, or two Saturdays per month, or any combination of one Saturday and one Sunday per month, at any of Corporation's practice locations; however, additional weekend rotation days could be scheduled from time to time based on operational and clinical needs of the Corporation as determined by Corporation in its sole discretionParticipate in holiday rotation, working 1-2 holidays per year.
