MSW Social Worker

Job Summary:


We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required.

Responsibilities:

• Perform comprehensive patient assessments and develop individualized treatment plans.
• Administer medications and treatments as prescribed.
• Provide skilled nursing care in various settings including acute medical hospital environments.
• Collaborate effectively with interdisciplinary teams to coordinate patient care.
• Manage casework and provide hospice care for patients with serious illnesses.
• Maintain accurate and timely documentation of patient care and clinical observations.
• Communicate clearly with patients, families, and healthcare providers regarding care plans and progress.

Required Skills & Certifications:

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of 2 years hospital, home health, or hospice care experience.
• Proficient clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and automobile for field work.

Preferred Skills & Certifications:

• Experience in acute medical hospital settings or specialized nursing units such as ICU or ER.
• Additional certifications relevant to specialty areas (e.g., Critical Care, Psychiatric Nursing).
• Familiarity with electronic medical records and clinical documentation systems.

Special Considerations:

• Weekend shifts required in rotation to meet departmental needs.
• Return to Office (RTO) restrictions limit work from home to no more than 7 days.
• Business casual dress code.

Scheduling:

• Work schedule includes weekend rotations.
• Shift assignments based on departmental needs and patient census.
• Typical patient census ranges from 40-50 daily.
