Lead Care Manager

Managing a direct patient caseload, the full-time California Licensed Lead Care Manager will oversee clinical performance and compliance of a team of Care Managers while maintaining remote patient monitoring and chronic care management standards. Key Responsibilities Manage a defined remote patient monitoring and chronic care management patient panel, ensuring compliance and productivity standards are met Conduct regular coaching sessions with Care Managers, utilizing performance data to enhance team outcomes Lead client-facing engagements, representing the organization with clinical authority in performance reviews and care coordination discussions Required Qualifications Bachelor's degree in nursing or a closely related healthcare field Active and valid registered nurse (RN) license in good standing Minimum of three to five years of clinical experience in care management or chronic disease management Prior experience in a lead or supervisory clinical role with accountability for team performance Working knowledge of remote patient monitoring and chronic care program documentation requirements
