Field-Based Peer Counselor II Community Health Advocate
NYC Health + Hospitals
Elmhurst Hospital Center in New York City is seeking a Peer Counselor to support health consumers across emergency, inpatient, outpatient and home settings. You will collaborate with clinicians to plan and implement treatment and empower clients to advocate for themselves. Responsibilities include documenting in EHR, coordinating referrals, and building peer support networks within the community. A formal peer training and state certifications are required, with preference for CPEP/substance-use #J-18808-Ljbffr
