RN Case Manager - Home Health

Location: Plainsboro, NJ and West Windsor, NJ

Assignment Duration: This is a long-term contract position.

Schedule: 8:00 AM - 4:30 PM

Job Description

We are seeking an experienced RN Case Manager for our Home Health team. The role requires a dedicated professional with a strong background in case management and OASIS documentation. The successful candidate will manage a high volume of patients and documentation, ensuring quality care in the home health setting.

Responsibilities

• Conduct comprehensive assessments and develop care plans for patients.
• Document patient progress and compliance with care plans.
• Coordinate care with interdisciplinary teams and facilitate transitions of care.
• Oversee 5-6 patient visits daily with a focus on quality and documentation.
• Provide wound care and treatment as needed.

Required Experience / Certifications / Licensure

• Minimum of 2 years of home health experience.
• Experience in case management and OASIS is a must.
• Wound care experience is preferred.
• Current RN license in the state of New Jersey.
