Pay: (Based on experience) + Benefits

Flexible Schedule / Part - time / Full - time

Schedule Options

Part-Time

• 2–3 full days per week
• Travel between Hoosick Falls, NY and Cambridge, NY

Full-Time

• 4–5 full days per week
• Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY

Job Overview

We are seeking an experienced Nurse Practitioner (NP) or Physician (MD/DO) to provide primary care services to residents in assisted living facilities throughout the Albany, New York region. This role offers flexible scheduling with both part-time and full-time options and is ideal for providers who enjoy building long-term relationships with geriatric patients.

Responsibilities

• Provide comprehensive primary care services to assisted living residents.
• Perform patient assessments, diagnoses, treatment planning, and follow-up care.
• Manage chronic conditions and preventative healthcare needs.
• Collaborate with facility staff, families, and interdisciplinary healthcare teams.
• Maintain accurate and timely clinical documentation.
• Develop meaningful, long-term relationships with residents and caregivers.

Required Qualifications

• Active New York State Nurse Practitioner or Physician (MD/DO) license.
• Active DEA registration.
• Currently enrolled and credentialed with both Medicare and Medicaid.
• Primary care experience required.
• Nurse Practitioners must meet New York State independent practice requirements (minimum 3,600 clinical practice hours).

Preferred Qualifications

• Geriatric care experience.
• Assisted Living, Long-Term Care (LTC), or Skilled Nursing Facility (SNF) experience.
