As a Social Worker, you'll work closely with residents, families, nursing, therapy, and other members of the interdisciplinary team to assess needs, support care planning, coordinate discharge services, and advocate for resident rights. You'll play an important role in helping residents adjust to the skilled nursing environment while supporting their dignity, independence, and overall well-being.

What You'll Do

• Develop comprehensive social histories and psychosocial assessments for residents.
• Assist with individualized discharge and placement planning based on resident needs, preferences, and available resources.
• Complete assigned portions of the MDS and resident care plan accurately and within required timeframes.
• Participate in interdisciplinary Care Plan meetings and contribute information regarding residents' psychosocial needs and progress.
• Document progress notes related to resident care plans according to facility policies and regulatory requirements.
• Assist residents and families in understanding discharge options, community resources, and available support services.
• Participate in pre-admission screening and provide information and support to prospective residents and families.
• Welcome and orient new residents to the skilled nursing environment and help facilitate a positive adjustment to placement.
• Advocate for resident rights, dignity, choice, privacy, and independence.
• Build positive relationships with residents, families, visitors, associates, community partners, and regulatory representatives.
• Provide education regarding aging, resident rights, facility services, community resources, and other relevant topics.

What Makes You Successful

• Bachelor's degree in Social Work or a related field as permitted by applicable state and federal requirements.
• Previous social work, human services, senior living, skilled nursing, or healthcare experience preferred.
• Knowledge of psychosocial assessment, care planning, discharge planning, and social work methods.
• Strong communication, documentation, organization, and interpersonal skills.
• Ability to communicate effectively and compassionately with residents and families.
