Work Environment

This position is onsite, primarily centered around home care. As a Physical Therapist, you will work directly within the clients' homes, allowing you to assess their living conditions and deliver therapy in a familiar environment. Key Responsibilities

Your role as a Physical Therapist requires you to:

• Direct and implement physical therapy treatment plans aimed at preventing disability and relieving pain while enhancing muscle function and maintaining optimal performance.
• Oversee and mentor Physical Therapy Assistants to ensure effective implementation of the care plan.
• Conduct thorough evaluations, using diagnostic tests to assess clients' functional abilities in muscle, nerve, and joint performance.
• Analyze home environments to recommend modifications that enhance clients’ independence.
• Administer therapeutic exercises and physical treatments such as traction and massage, providing motivation and guidance to clients through their therapy journey.
• Evaluate and adjust prosthetic and orthotic devices, working closely with physicians to ensure optimal patient care.
• Participate in interdisciplinary team meetings to collaboratively assess and review care plans, making necessary adjustments based on client progress.
• Maintain comprehensive clinical records, including initial evaluations, care plans, and progress notes, in accordance with regulatory standards.
• Regularly communicate with clients, families, and agency personnel to ensure a cohesive approach to care.
• Continuously monitor and report any changes in clients’ conditions, advocating for adjustments in treatment plans when necessary.
• Support the need for ongoing assessment and utilization of community resources to facilitate effective rehabilitation strategies for clients.

Patient Population / Client Focus

Our clients encompass a diverse demographic, including seniors, individuals recovering from surgery, those with chronic illness, and patients in need of post-operative rehabilitation. You will support clients in their own homes, promoting autonomy through customized care plans that match their health goals and rehabilitation needs.

Qualifications

The ideal candidate will possess the following qualifications:

• Graduated from an accredited physical therapy program with a Doctorate in Physical Therapy.
• Current licensure as a Physical Therapist in the state of Virginia, maintaining good standing.
• CPR certification and a valid driver’s license with automobile insurance.
• A passion for delivering high-quality healthcare that embraces clinical excellence and patient engagement.
• Strong interpersonal communication skills, coupled with a positive attitude and ability to collaborate effectively with healthcare teammates.

Preferred Experience

While the baseline requirement is one year of experience in physical therapy within acute care or home care settings, candidates with demonstrated leadership or specialized therapeutic skills will be prioritized. Familiarity with electronic health record (EHR) systems and proficiency in federal and state regulations governing physical therapy practice are preferred.
