Recovery Support Counselor Mon-Fri, Community-Based

Trilogy

Trilogy in Chicago seeks a Peer Support to assist clients with mental health and co-occurring disorders. You will perform home, community, and office visits, develop recovery plans, provide trauma-informed, strengths-based support, and act as an advocate linking clients to community resources. Responsibilities include completing daily progress notes, crisis/on-call coverage, coordinating with nurses and pharmacies, and helping with independent living skills. #J-18808-Ljbffr
