Impactful Community-Based Peer Recovery Counselor

TRILOGY BEHAVIORAL HEALTHCARE is seeking a PeerSupport to assess clients across personal, medical, emotional, social, and environmental needs and plan treatment through home, community, and office visits. The role provides client-centered, strengths-based, trauma-informed services and links clients to community resources within a harm-reduction framework.
The PeerSupport will maintain a shared caseload, complete timely progress notes, participate in team meetings, and provide crisis/on-call
