Woodland Manor is seeking a PRN Physical Therapist Assistant (PTA) to provide high-quality rehabilitation services at our skilled nursing facility in Elkhart, IN. This flexible, as-needed role is ideal for a dedicated clinician who wants to make a meaningful impact while maintaining schedule flexibility.

Your Responsibilities:

• Assist with providing evidence-based physical therapy services under the direction of a licensed Physical Therapist
• Implement personalized treatment plans designed to improve mobility, strength, balance, and functional independence
• Monitor resident progress and communicate changes or concerns with the Physical Therapist and interdisciplinary team
• Provide hands-on therapeutic interventions and support residents in achieving their functional goals
• Document care accurately and maintain compliance with clinical and regulatory standards

What You'll Bring:

• Current Indiana licensure as a Physical Therapist Assistant
• Graduate of an accredited Physical Therapist Assistant program
• BLS certification
• Excellent communication skills, clinical judgment, and a resident-centered mindset
