What You ll Do

• Perform comprehensive health assessments and physical exams

• Conduct medication reviews and chronic care management

• Develop individualized care plans with interdisciplinary teams

• Manage acute and chronic conditions confidently

• Order and interpret labs and diagnostics

• Prescribe medications within NP scope of practice

• Educate residents and families on preventive care and wellness

• Maintain accurate documentation in eClinicalWorks

• Participate in quality improvement initiatives

What You ll Bring

• Master s Degree from an accredited Nurse Practitioner program

• Active New York State NP license

• Active DEA registration

• Medicare/Medicaid billing eligibility

• Minimum 3 years of clinical experience

• Geriatric or long-term care experience preferred

• Strong bedside manner, compassion, and communication skills
