What You ll Do

Perform comprehensive health assessments and physical exams
Conduct medication reviews and chronic care management
Develop individualized care plans with interdisciplinary teams
Manage acute and chronic conditions confidently
Order and interpret labs and diagnostics
Prescribe medications within NP scope of practice
Educate residents and families on preventive care and wellness
Maintain accurate documentation in eClinicalWorks
Participate in quality improvement initiatives

What You ll Bring

Master s Degree from an accredited Nurse Practitioner program
Active New York State NP license
Active DEA registration
Medicare/Medicaid billing eligibility
Minimum 3 years of clinical experience
Geriatric or long-term care experience preferred
Strong bedside manner, compassion, and communication skills
