Although Willamette is priority. Provider will be partially in house for 6-8 hours a day which will be billed at the regular hourly rate (7am-1pm or 7am-3pm) then the rest of the day will be billed as night call. They will be on beeper call 1pm or 3pm until 7am the next morning. They must be within 30 minutes of the hospital. Please note this will be billed as regular hours with night call on the week days. • Lunch N/A • Provider Min Shifts Required: July - 5 shifts, August - 16 shifts, ongoing we will need 10 shifts a month about. • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 8 hours per day • Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. ________________________________________________________________ CREDENTIALING • Current CV (must be uploaded to bid) • License: Active OR • DEA Required: Yes • Boards: BC only • Skills/Certs: BLS, NRP & Stable • Other: ________________________________________________________________ ADDITIONAL DETAILS Dates Needed: July - 5 shifts, August - 16 shifts. Ongoing we will need about 10 shifts a month and will look at 1-2 providers. There is flexibility in the schedule, but typically the shifts are not in a row and sporadic days. The site may be able to move dates around so they are paired together. Provider should have resuscitation skills (there are about 1-3 a week) Newborn rounding skills They will be partial in-house (6-8 hours) and then on call the rest of the day. It is beeper call (and be within 30 minutes of the hospital). Less than 25% call back rate. Clean file is required for temp privileges. Can not work another site while scheduled here. Provider will need 2 years of case logs. Required: Intubation of Newborn, UVC, Needle Pneumothorax (decompression), x ray interpretation.
