JOB DESCRIPTION Key Responsibilities: Conduct patient evaluations, physical exams, and follow-up visits. Diagnose and manage acute and chronic cardiovascular conditions. Order and interpret diagnostic tests and imaging. Develop treatment plans, prescribe medications, and coordinate care. Educate patients and families on disease management and treatment plans. Collaborate with physicians and the multidisciplinary team to optimize outcomes. Maintain accurate and timely documentation. Preferred Experience: Heart Failure or Advanced Heart Failure Cardiology or Cardiovascular Medicine Critical Care Electrophysiology Cardiothoracic Surgery QUALIFICATIONS Required: Master's degree as a Nurse Practitioner or Physician Assistant from an accredited program. Current state licensure and certification as a Nurse Practitioner or Physician Assistant. Preferred: Five or more years of relevant clinical experience in a healthcare setting or specialty practice.
