Typical responsibilities include conducting initial and ongoing nursing assessments in the home; developing and updating individualized plans of care; coordinating with physicians and other team members to ensure appropriate medication and symptom management; educating patients and families about disease progression, comfort measures, and available support services; and providing direct hands-on care such as wound care, medication administration, and comfort-focused interventions. You will also participate in regular interdisciplinary team meetings to review patient status, contribute to care planning, and ensure continuity of care. Your day may include traveling between patient homes across an assigned region of Orange County, performing scheduled visits, and responding to urgent needs as appropriate. Visit volumes and patient caseloads are designed to allow for thorough, supportive interactions rather than rushed encounters, with an emphasis on holistic care that considers physical, emotional, and spiritual needs. Shifts typically follow daytime or early evening hours, with some flexibility based on patient needs and agency scheduling structures.
