Description

American Traveler is hiring a Home Health RN Float to serve the North Region, requiring an active MA RN license and at least one year of certified home health agency experience.

Details

• Home health setting covering Chelsea, East Boston, Revere, and Winthrop
• Float role requiring travel to patient cases as assigned across the service area
• 13-week assignment with a schedule of 08:00-16:30
• Weekend rotation required once every six weeks
• Uses Epic for electronic documentation and charting

Requirements

• Active MA RN license required, no conditional or pending licenses accepted
• AHA-issued certifications only
• Minimum 1 year of experience within a certified home health agency (hospice, private care, or standalone case management roles do not qualify)
• Minimum 1 year of case management experience
• Flu vaccination required during flu season with no medical or religious exemptions allowed
• Epic charting experience required

Additional Information

• Responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and families
• Performs admission processes and case management duties in accordance with home care policies and standards
• Delivers care aligned with organizational philosophy and community health practice standards
• Requires thorough, timely electronic documentation and interdisciplinary communication
• Local candidates considered if permanent residence is within 60 miles of Massachusetts General Hospital in Boston
