What You'll Do

As a Home Health Physical Therapist, you will:

• Evaluate and treat homebound patients referred for physical therapy services
• Develop individualized treatment plans designed to maximize patient independence and function
• Assess strength, balance, mobility, endurance, gait, pain, and activities of daily living
• Deliver evidence-based therapeutic interventions and exercises
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Educate patients and caregivers to support recovery and long-term success
• Monitor progress and adjust treatment plans as needed

Qualifications

Required

• Graduate of an accredited Physical Therapy program (Bachelor's or Entry-Level Master's Degree)
• Active Connecticut Physical Therapist License
• CPR Certification (required by start date)
• Prior Physical Therapy experience
