Call Schedule: shared family medicine call group.Be part of something remarkableAs an Advanced Practice Provider, you'll play an integral role in our ambulatory care teams by providing high-quality, cost-effective care to patientsWe are seeking a Nurse Practitioner OR Physician Assistant to:Be a Primary Care Provider (PCP)Care for the acute and chronic needs of patients from newborn to elderly.Perform annual physical exams and manage the health maintenance needs of our family medicine population.Perform routine clinical procedures independentlyEducation:If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program.If Physician Assistant: Bachelor's or Master's degree from accredited PA Program.Work Experience:Requires 3 years or 3600 hours of primary care experienceLicenses and Certifications:Minimum – State of Wisconsin NP or PA licensureIf Nurse Practitioner*:Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice ANDActive board certification as a NP in area of clinical practice.
