We're currently seeking a full-time Field Registered Nurse to support clients throughout Binghamton/Syracuse Region in Central New York. This role offers the opportunity to build meaningful relationships with clients while providing clinical oversight and care in the comfort of their homes.

What You'll Do:

• Provides medical oversight for clients in their homes- including but not limited to assessments, obtaining MD orders, monthly contacts, and maintaining an appropriate Plan of Care.
• Communicate with referrers, discharge planners, and physicians to ensure seamless patient care.
• Develop, implement, and monitor individualized treatment plans, ensuring proper care coordination and medication management.
• Perform in-home patient assessments, reassessments, and provide intermittent nursing services.
• Ensure accurate documentation and compliance with agency policies, including patient rights and discharge planning.
• Supervise and provide in-service education for staff, conducting competency evaluations.
• Participate in quality improvement programs and policy reviews.
• Provide 24-hour on-call nursing coverage on a rotating basis.
• May assist with PCA Training Program as Needed.
