Nursing Care Coordinator (RN/LPN) - On-Site Day Shift

Infinite Medical P.C. seeks a detail‑oriented on‑site Nursing Care Coordinator (RN or LPN) to verify chart documentation and ensure CMS compliance. This role supports continuity of care for our senior patient population and works with HR, Operations, and Clinical teams to resolve inquiries. The position emphasizes care management, chronic disease understanding, and accurate documentation across multiple EMR systems, with a standard 9:00 AM–5:30 PM schedule and a focus on quality and #J-18808-Ljbffr MedElite Group, LLC.
