Licensed Practical Nurse - Full Time - Connecticut Pediatrics @ CHC - Hartford

Job Description Summary Monday – Friday with one Saturday day a month. On a daily basis, provides clinical and technical support for primary care providers. Implements all planned care items using electronic tools to ensure efficient and accurate workflow. Monitors equipment and performs clinical testing according to OSHA and CLIA standards. Liaises with patients, families, specialists, and all members of the health care team. With the primary care nurse and provider, ensures continuity and coordination of care in a patient centered medical home. Assists the primary care nurse as able with routine medication administration, immunizations and other provider clinic support supported by the scope of LPN licensure. Role and Responsibilities Apply evidence based guidelines from major regulatory and specialty groups, perform pre-visit preparation for all primary care patients scheduled to be seen. In collaboration with the primary care nurse and provider, conduct e-huddles utilizing the Planned Care Dashboard to ensure complete and thorough primary care visits. Prepare patients to be seen by provider, using established criteria as appropriately determined by the nature of the visit (e.g., sick, PE, WCC, etc.). Accurately document chief complaint and results from evidence based screening tests. Accurately perform and document patient vital signs. Independently perform appropriate CLIA waived tests, EKG, hearing and vision test. Prioritize work responsibilities on a daily basis, decreasing waiting time. Collaborate with providers, clinical and clerical support staff to facilitate appropriate patient flow. Effectively work as a member of the health care team assisting other team members as needed or requested. Appropriately schedule tests and referrals for specialty care in accordance with provider orders in a timely manner. Maintain a clean, orderly, safe working environment. Oversee maintenance of supplies and documents in medical logs as directed. Set up and stock exam rooms and clean rooms at end of each day worked. Adhere to all infection control standards, follow universal precautions and safety policies and procedures of the agency. Maintain yearly CPR certification as required for all clinical employees. Exhibit excellent customer service skills to external and internal customers at all times with a focus on quality of care and teamwork. Comply with agency mandated in-service training workshops (EOC, Working Safely, Abuse, etc.). Assist the primary care nurse with provider clinic support such as routine medication administration, immunizations, and other items as able. All medications and immunizations should be administered under the delegation of CHCI's standing order, or Primary Care RN or Primary Care Provider delegation. Qualifications Required Skills and Education: LPN Program Completion Certificate. Skills include EKG use, appointment scheduling, word processing, computer skills, and familiarity with medical testing procedures. Applicant must possess sound organizational skills, accuracy at all levels of the job, excellent communication and customer service skills, be team focused, and demonstrate leadership qualities. Preferred Skills: Bilingual. Expert knowledge of the electronic health record. Required Licenses/Certifications: Current CT LPN License; CPR – American Red Cross CPR for the Professional Rescuer and AED certification or American Heart Association BLS/AED for Healthcare Providers. Physical Requirements / Work Environment Requires manual dexterity to perform clinical tasks, moderate physical activity including standing, walking and lifting. Potential exposure to blood borne pathogens and/or bio-hazardous material. Frequent and alternating standing, walking and sitting. Occasional bending, squatting, kneeling and lifting up to 25 pounds. Daily and continuous computer use. Daily and frequent telephone use. Additional Qualifications Communicates clinical/client information to providers accurately. Accurately take messages, communicate on the telephone. Communicates effectively and collaborates well with other staff members including medical assistants, nurses and providers. Access to clients’ charts and health care information. Maintains client confidentiality. Committee membership participation in quality improvement projects. Precepting experience.
