Requirements Applicants must have an active Kentucky Physical Therapist license or an active Kentucky Physical Therapist Assistant license or must have the necessary qualifications to earn Kentucky license. These qualifications include earning a degree from a Commission on Accreditation in Physical Therapy Education‑accredited physical therapist or physical therapist assistant education program, passing the state licensure exam, and fulfilling the state’s requirements for licensure.
