Housing Case Manager

City Net in Santa Barbara is seeking a Case Manager (Homeless Outreach) to provide housing-focused case management and supportive services through street outreach. You will meet clients where they are, helping develop long-term housing plans and connecting them to medical, mental health care, counseling, and permanent housing. Work schedule is 4/10s, Monday-Thursday, 7:30am-6:00pm, with emphasis on intake, housing placement, coordination with partners, and data entry in HMIS/CES systems to #J-18808-Ljbffr
