Hospice And Home Health Social Worker

Provide emotional, social, and practical support to hospice and home health patients while coordinating community resources and ensuring holistic, patient-centered care.

Key Responsibilities:

• Conduct psychosocial assessments for hospice and home health patients
• Provide emotional support, counseling, and crisis intervention
• Assist patients and caregivers with advance care planning and end-of-life support
• Connect patients with community resources such as Medicaid assistance, meals on wheels, transportation, caregiver support, utility assistance, and housing support
• Coordinate respite care services and hospice transfers as needed
• Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
• Advocate for patient rights, dignity, and quality of life
• Maintain accurate documentation in compliance with regulations

Qualifications:

• Bachelor's or Master's degree in Social Work from an accredited program
• Active Idaho Social Work license
• Licensed Master Social Worker (LMSW) required
• Knowledge of community resources, Medicaid, and social service systems
• Strong communication, organization, and time-management skills
• Valid driver's license with ability to travel to patient homes
