What you'll do

• Evaluate patients' functional status, mobility, endurance, pain, and rehabilitation needs
• Develop individualized treatment plans in collaboration with physicians and the care team
• Deliver physical therapy services in accordance with the plan of care and state practice guidelines
• Help patients improve movement, function, strength, and independence
• Educate patients and caregivers on treatment goals, safety, and home exercise programs
• Monitor progress and communicate changes in condition to the care team
• Complete documentation and plan-of-care requirements in accordance with company policies

Required Qualifications

• Current Physical Therapy licensure in state of practice
• Current CPR certification
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Compensation

Annual Total Cash Target Pay: $82,048 - $123,072
Hourly Pay Range: $39.45 - $59.17
Per Visit Point Pay Range: $47.34 - $71.00
