Role Scope : The Licensed Vocational Nurse (LVN) supports the delivery of high-quality, participant-centered nursing care across Habitat Health's PACE care setting at our South LA PACE Center. Working within scope of practice and in partnership with Registered Nurses, providers, and the Interdisciplinary Team (IDT), the LVN provides direct nursing care, monitors participant health status, supports care plan interventions, and coordinates services that promote continuity of care. Through sound clinical judgment, proactive communication, and compassionate engagement, the LVN contributes to participant safety, quality outcomes, independence, and overall well-being.
Core Responsibilities & Expectations for the Role
- Provides direct nursing care and treatments within scope of practice and under appropriate clinical direction, including medication administration, injections, vital sign monitoring, wound care, and other ordered interventions.
- Conducts participant triage, nursing observations, and assigned assessments using established clinical protocols, escalating acute needs, changes in condition, and other clinical concerns to the RN or provider as appropriate.
- Implements individualized care plan interventions and supports chronic disease management, intake activities, and ongoing participant care in partnership with Registered Nurses, providers, and the Interdisciplinary Team (IDT).
- Coordinates participant care and transitions across the clinic, day center, home, external providers, and other care settings to promote continuity and timely access to services.
- Partners with participants, caregivers, RN Case Managers, providers, therapists, social workers, and other members of the IDT to communicate participant needs, integrate participant preferences, and support coordinated care planning and delivery.
- Provides participant and caregiver education on medications, nutrition, hygiene, chronic condition management, and other health needs while supporting participant autonomy, informed decision-making, and safety.
- Builds therapeutic relationships with participants through compassionate, respectful, and culturally responsive care that promotes engagement, dignity, independence, and quality of life.
- Maintains accurate, timely, and compliant clinical documentation within the Electronic Medical Record (EMR), including participant assessments, interventions, responses to treatment, and changes in health status.
- Contributes to quality, safety, infection prevention, and continuous improvement initiatives by following established clinical protocols and identifying opportunities to strengthen participant outcomes, documentation, workflows, and care delivery.
- Performs other related duties as assigned.
Required Qualifications:
- Completion of an accredited vocational or practical nursing program, or equivalent nursing education required for licensure in the applicable state.
- Active, unrestricted Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license, as applicable to the state of practice.
- Minimum of one (1) year of nursing experience, preferably supporting older adults, frail elderly populations, or individuals with complex medical and functional needs.
- Demonstrated ability to provide direct nursing care, recognize and communicate changes in participant condition, and appropriately escalate clinical concerns within scope of practice.
- Demonstrated ability to apply established clinical protocols while exercising sound judgment and maintaining participant safety, dignity, and preferences.
- Strong communication and collaboration skills, with the ability to effectively engage participants, caregivers, providers, and interdisciplinary team members to deliver coordinated, participant-centered care.
- Strong organizational skills with the ability to manage multiple clinical priorities while maintaining attention to detail and ensuring timely follow-through.
- Experience using Electronic Medical Records (EMRs) and the ability to quickly learn clinical documentation and other organizational systems.
- Demonstrated ability to work across care settings, including the clinic, PACE center, participant homes, and community settings, based on participant and operational needs.
- Active CPR/BLS certification or ability to obtain certification within 30 days of hire.
- Proof of a valid driver's license, reliable personal transportation, and automobile insurance that meets applicable state requirements, with the ability to travel throughout the service area.
- Meets all applicable participant-facing health requirements, including required communicable disease screening, TB clearance, and immunizations.

Compensation: The anticipated salary range for this position is $30.00 - $34.00 per hour . This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable. The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs. In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
