Now Hiring: Ohio Regional MDS Nurse

Full Time | HYBRID Schedule | $90,000-$110,000

Responsibilities:

• Oversee Ohio facilities MDS positions

• Travel required

• Conduct thorough assessments of residents' needs and develop individualized care plans to ensure their comfort, safety, and well-being

• Collaborate with interdisciplinary teams to identify and address resident concerns, and implement necessary changes to improve care quality

• Coordinate with MDS coordinators to ensure accurate and timely completion of Minimum Data Set (MDS) assessments

• Participate in quality improvement initiatives to drive positive change and enhance resident outcomes

• Provide education and support to residents, families, and staff on various aspects of care, including wound care, skin integrity, and infection prevention

• Foster a culture of compassion, respect, and open communication among residents, families, and staff

Requirements:

• Current RN licensure in the State of Ohio

• Minimum 1 years of experience in a long-term care setting, with a focus on MDS assessments and care planning

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities

• Strong communication and interpersonal skills, with the ability to build trusting relationships with residents, families, and staff

• Ability to travel to Ohio facilities 2-3 days per week.
