Your Next Adventure: Regional Nurse Practitioner/Physician Assistant Reporting To: Director of Clinical Operations Location: San Jose, Mountain View, Campbell Classification: Regular, Full-Time, Non-Exempt Schedule: Tuesday - Friday in Mountain View, every other Saturday in San Jose or Campbell; 6:50am - 2:00pm, some later days required due to internal meetings or administrative tasks

We are seeking an individual with an unwavering dedication to exceptional patient care, a collaborative spirit, intrinsic motivation for helping others, and an acute attention to detailall complemented by a polished and professional demeanor. The ideal candidate should be ready and willing to lead our small yet formidable team of health coaches and member care coordinators.

Duties & Responsibilities

• Conduct 1:1 member consultations for weight loss, applying the JumpstartMD program.
• Provide routine patient care, including examinations, screenings, and medication administration.
• Ensure up-to-date patient records, ordering and evaluating lab tests as needed.
• Conduct follow-up calls and promptly respond to post-visit inquiries.
• Adhere to company policies, safety regulations, and productivity metrics in delivering quality patient care.
• Support team by assisting with medical and program-related questions, attending meetings, and completing required training.

Qualifications and Requirements

• California licensed Nurse Practitioner or Physician Assistant.
• Master's degree from an accredited NP or PA program.
• DEA Registration Certificate.
• OIG Clearance and NPI number
• Must be able to meet and receive a background check to include criminal records clearance.
