Job Overview The Care Manager / Designated Care Manager provides the highest degree of quality care and services to a consistent group of residents and their families in assisted living and long‑term care and reminiscence neighborhoods. The role includes hands‑on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), while maintaining a safe, comfortable home‑like environment in accordance with federal, state, provincial and local laws and Sunrise Senior Living standards. Responsibilities Review, notate and initial the Daily Log to document resident information and monitor changes in skin, behavior, alertness, weight, diet and activity. Participate in the development and monthly updates of ISPs and review designated assignments for residents. Maintain a designated group of residents during the shift, conduct physical checks and observe/report symptoms and condition changes. Attend daily Cross‑Over meetings with the lead care manager and promptly notify supervisors of increased care needs or resident condition changes. Respond immediately to security system and resident call bells, resetting call bells as needed. Greet guests, family members, residents and team members and handle phone calls courteously and professionally. Assist with continence management and dispose of products to ensure sanitation of resident suites and community restrooms. Communicate with families as a resource and support resident independence, safety and dignity. Lead and assist residents with activities of daily living (ADLs) and encourage participation in activities indicated in the ISP. Engage residents in life skills and reminiscence activities throughout the day, fostering empathy and understanding of each resident’s communication patterns or history. Maintain an atmosphere of warmth, personal interest and positive emphasis while ensuring established safety regulations are followed. Practice routine standard care precautions of cleanliness, hygiene, and health. Partner with the community team to ensure compliance with national and provincial health and safety regulations, risk‑management programs and policies. Maintain safety procedures including PPE, fire extinguishers, safety data sheets, lockout‑tagout procedures, and report all accidents or unsafe conditions immediately. Manage housekeeping tasks: clean resident rooms, make beds, empty trash, sweep, mop, wash laundry and linens, and keep common areas tidy. Serve meals in the dining room, promote a pleasant dining experience, assist with set‑up and clean‑up, follow resident diet needs and document changes in the daily log. Provide room‑service delivery as needed and practice safe food handling in compliance with universal care precautions. Support planning and execution of resident enrichment activities, including “Creating Pleasant Days” and monthly letters to homes. Participate in team meetings, training modules, engagement surveys and workshops; maintain compliance with infection control and required training. Perform other duties as assigned by supervisors or department coordinators. Experience and Qualifications High School diploma or GED (may be required by state/provincial regulations). CPR and First Aid certification, as required by state/provincial regulations. Minimum age 18 years. Previous experience working with seniors preferred. Demonstrated desire to serve and care for seniors and ability to make choices and decisions in the resident’s best interest. Power of attorney to drive a Sunrise vehicle requires additional signing of a driver job description and compliance with safety regulations. Core Competencies Ability to remain calm in difficult situations. Ability to handle multiple priorities simultaneously. Strong written and verbal communication skills with a high level of understanding. Competency in organizational and time‑management skills. Good judgment, problem‑solving and decision‑making skills.
