Supportive Services Case Manager

Five Keys Schools and Programs

Job Summary The Supportive Services Case Manager provides housing‑focused case management to San Francisco clients receiving rapid rehousing rental assistance through the San Francisco Department of Housing and Homeless Services and other city departments. The position reports to the Five Keys rehousing program team at 821 Folsom Street, San Francisco. The role is full‑time and temporary, anticipated duration three to six months. Job Duties And Responsibilities Case Management Duties Provide housing navigation assistance, including accompaniment and transportation to view housing units, support with applications, credit/background reports. Help participants understand tenants’ rights, rent payment, communication with landlords, lease responsibilities. Offer support services to stabilize living situations, improve mental and physical health, increase independence. Develop case management plans and connect clients with community and government resources to avoid loss of benefits. Make referrals and coordinate services that lead to housing stability, income increase, job placement, retention. Collaborate with the rental assistance provider to calculate assistance. Support participants transitioning off the Rapid Rehousing Rental Assistance program. Plan and facilitate group Life Skills sessions covering budgeting, finances, housing search, nutrition, landlord communication, health awareness, and parenting. Maintain protocol and records of client feedback and grievances. Document case records, review reports, compile data, and prepare summary reports meeting Five Keys and HSH standards. Administrative Duties Input client data and outcomes into Five Keys systems (Apricot), the Online Navigation and Entry (ONE) system, and submit reports. Provide monthly and quarterly client activity reports to the Assistant Director of Housing Stability Programs. Manage enrollments, assessments, contact logs, unit service records, referrals, and case notes. General Duties Provide a welcoming environment for all clients and colleagues. Coordinate with CBOs and government organizations for client resources. Support all administrative tasks related to Five Keys Rehousing Programs. Respond to information requests from Five Keys Administration. Maintain accurate records of referrals in Clarity and Apricot. Document programmatic protocol violations with incident forms. Abide by Five Keys and HSH policies and procedures. Ensure quality assurance of programming and overall case management. Perform other duties as assigned. Knowledge and Skills Knowledge of recovery models, including harm reduction. Comfortable, mature, and professional interactions with diverse partners and participants. Excellent communication skills, both oral and written. Proficiency with computers, Microsoft Office, databases, intranet, and email. Strong data entry skills and ability to learn new systems. Willingness to grow professionally and adapt to evolving environments. Commitment to Five Keys mission and participant needs. Ability to maintain boundaries with personal identifying information. Exceptional written communication skills. Excellent interpersonal and organizational skills. Capacity to manage multiple deadlines and tasks simultaneously. Advanced problem‑solving skills; ability to resolve questions/concerns. Proven case‑management and group facilitation experience. Preferred: proven record for completing administrative tasks. Education and Work Experience Minimum five years of direct service experience with similar populations. Typically 2–3 years working with homeless and young adult populations, or an equivalent combination of experience and education. Preferred experience working with unhoused populations or similar to Five Keys students. Physical Requirements Must be fully vaccinated against COVID‑19 or have an approved exemption. Wear provided PPE and follow safety guidelines. Frequent speaking and hearing conversations. Long periods of walking, sitting at a desk, or using a computer. Lifting up to 25 lb; bending, reaching, carrying, stooping during routine tasks. Additional Requirements Submit proof of clear TB results dated within 60 days prior to hire. Work Environment Willing and able to work in a correctional facility and travel to community sites. Moderate to high noise level. Variable work environment across sites.
