About the Role:

The Care Partner plays a vital role in supporting individuals with their daily living activities to enhance their quality of life and promote independence. This position involves providing compassionate, respectful, and personalized care tailored to each individual's unique needs. The Care Partner collaborates closely with healthcare professionals and family members to ensure a safe and comfortable environment. Through attentive observation and communication, the Care Partner helps identify changes in health status and reports them promptly to the appropriate team members. Ultimately, this role contributes significantly to the physical, emotional, and social well-being of those under their care.

Minimum Qualifications:

• High school diploma or equivalent.
• Previous experience in a caregiving or support role preferred but not required.
• Ability to pass background checks and any required health screenings.
• Strong communication skills and the ability to follow instructions accurately.
• Physical capability to assist with mobility and perform caregiving tasks safely.

Preferred Qualifications:

• Certification in CPR and First Aid.
• Formal training or certification as a Certified Nursing Assistant (CNA) or equivalent.
• Experience working with elderly, disabled, or chronically ill individuals.
• Knowledge of basic health and safety regulations in caregiving settings.
• Familiarity with electronic health record (EHR) systems or caregiving documentation tools.

Responsibilities:

• Assist individuals with activities of daily living such as bathing, dressing, grooming, and mobility support.
• Monitor and document changes in physical or emotional condition and communicate observations to healthcare staff.
• Provide companionship and emotional support to promote mental and social well-being.
• Ensure a clean, safe, and comfortable living environment by performing light housekeeping and organizing personal spaces.
• Support medication reminders and assist with meal preparation and feeding as needed.

Skills:

The Care Partner utilizes strong interpersonal and communication skills daily to build trust and rapport with individuals receiving care and their families. Attention to detail and observational skills are essential for monitoring health changes and ensuring safety. Physical stamina and coordination are required to assist with mobility and personal care tasks effectively. Time management and organizational skills help the Care Partner balance multiple responsibilities and maintain a clean, orderly environment. Additionally, empathy and patience are critical in providing emotional support and fostering a positive caregiving experience.
