Population Health RN - Chronic Care Manager (Ambulatory)

Greater Baden Medical Services, Inc. is seeking an Ambulatory Care RN Population Health/Chronic Care Manager to lead care coordination, prevent readmissions, and advance preventive care across our Maryland clinics. This role supports PCMH initiatives and acts as a clinical resource for safe, efficient patient care. The ideal candidate will use the nursing process, educate patients, manage chronic conditions, and collaborate with providers, staff, and community partners to improve population #J-18808-Ljbffr Greater Baden Medical Services, Inc.
