Job Summary:

SunnyBrook HomeCare, a home health agency dedicated to the health and well-being of our patients, is seeking a compassionate and skilled Home Health Licensed Practical Nurse LPN to join our team! As a key member of our care team, you will provide high-quality patient care in a home health setting, covering Fort Madison, Burlington, and Keokuk areas.

Responsibilities:

• Travel to patients' homes to provide skilled nursing care, including wound care, medication management, and education
• Develop and implement individualized patient care plans in collaboration with the healthcare team
• Conduct patient assessments, evaluations, and progress reports to ensure optimal health outcomes
• Educate patients and their families on disease management, medication use, and self-care techniques
• Maintain accurate and detailed records of patient care and progress
