Schedule:

• Flexible shifts, Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

In this position, you will:

• Conduct comprehensive annual home health risk assessments in patients homes
• Review and evaluate prior and current medical history, conditions, and current medications
• Offer education to patients regarding their medical backgrounds and prescriptions
• Detect any gaps in care, providing practical recommendations to address them
• Visits may include annual health reviews or post-discharge follow-ups (approximately 45 minutes per evaluation)
• No development of treatment plans or medication prescribing required

Qualifications sought:

• Active Nurse Practitioner (NP) license in Maine
• Board Certified via AANP or ANCC
• MSN degree from an accredited nursing school
• Current BLS/CPR certification
• At least 6 months of recent nurse practitioner experience is strongly preferred
