Experienced nurse practitioners with a passion for delivering high-quality patient care are invited to provide home health risk assessments for Medicare Advantage plan members. As part of this per diem opportunity, you will complete in-home visits throughout various locations in York County, including Biddeford, Saco, and Sanford. This role offers unparalleled flexibility, allowing you to customize your schedule to best fit your commitments and availability.

Schedule:

• Flexible shifts available between Monday and Sunday
• Opportunity to create your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments (approximately 45 minutes each) for Medicare Advantage plan members
• Evaluate medical history, current conditions, and medication profiles
• Educate patients about their health conditions, wellness, and medications
• Identify existing gaps in care and recommend actions for improved patient outcomes
• Focus exclusively on assessmentsno medication prescribing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license in ME
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS (CPR certification)
• At least 6 months of nurse practitioner experience preferred
