This application allows us to gather required employment information to support your transition. Please ensure your employment history is complete and accurate, as it will be used for future years of experience determinations. This position is for TCMC Employees with a professional license that may include RN, NP, PA, PT, OT, Speech, Pharmacist, LSW, LFMT, etc. Please make sure to upload a copy of your license and any additional certifications that may apply to your role. **Will there be an interview as part of this application process?** No. There is no interview associated with this application. Your information is being collected to allow Sharp HealthCare to create your employment profile and for record keeping purposes. **Why is it important to complete the application fully?** It is very important that all employees complete the application fully and accurately. This information will be used as part of the transition into Sharp HealthCare's systems. **What work history should I include in my application?** Employees are asked to include their full work history, not just recent positions or roles related to their current job. Sharp HealthCare uses work history to assess years of experience for each employee. **Will my information be verified?** Employment verification may be conducted as needed. It is important to ensure that all information, including dates of employment and employer names, is complete and accurate. **Other Qualification Requirements*** Current, active professional license (as applicable to role) in good standing**Essential Functions*** Under the direction of appropriate clinical or professional leadership, the Professional Licensed Employee performs job-related duties consistent with their licensure, education, training, and scope of practice. This role supports patient care and/or professional services at Tri-City Medical Center while transitioning employment to Sharp HealthCare. \* Perform duties appropriate to professional license, certification, and assigned department \* Deliver safe, high-quality services in accordance with regulatory requirements, organizational policies, and professional standards \* Accurately document work performed in applicable systems \* Collaborate effectively with physicians, leaders, and interdisciplinary team members \* Support patient safety, quality initiatives, and service excellence \* Comply with all Sharp HealthCare and Tri-City Medical Center policies and procedures \* Participate in required training, education, and transition-related activities**Knowledge, Skills, and Abilities*** Ability to work effectively in a fast-paced healthcare environment* Strong communication and teamwork skillsSharp HealthCare is an equal opportunity/affirmative action employer.
