Evening Shift Licensed Practical Nurse (LPN)

As an Evening Shift Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering exceptional patient care while making a positive impact on our residents' lives.
Job Summary:
We are seeking a dedicated and compassionate LPN to join our evening shift team. As an LPN with Bradford Manor Nursing and Rehabilitation, you will provide high-quality nursing care to our residents, participate in assessments, develop and implement individualized care plans, and collaborate with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and assisting with ADLs
Conduct initial and ongoing assessments to identify resident needs and develop individualized care plans
Implement care plans, monitor progress, and communicate with residents, families, and healthcare team members
Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
*Maintain accurate and timely records of resident care and progress
*Participate in Quality Assurance and Performance Improvement initiatives
*Contribute to a positive and supportive work environment
Requirements:
Current LPN licensure in the state of Pennsylvania
High school diploma or equivalent required; LPN diploma or degree preferred
Previous LPN experience in a long-term care setting preferred
Ability to work effectively in a fast-paced environment with minimal supervision
Excellent communication and interpersonal skills
Strong attention to detail and organizational skills
Ability to maintain confidentiality and adhere to HIPAA guidelines
