About the Role:

We are seeking a dedicated and caring individual to provide non-medical assistance to our patients with daily living activities, including meal prep , medication reminders , bathing , and medication reminders . As a Home Health Aide, you will work closely with our care team to develop personalized care plans that cater to each patient's unique needs and preferences.

Responsibilities:

• Provide non-medical assistance with daily living activities, including meal prep , medication reminders , bathing , and medication reminders .

• Assist patients with grooming, dressing, and personal hygiene.

• Help patients with mobility and transfers, as needed.

• Monitor vital signs and report any changes to the care team.

• Maintain accurate and detailed records of patient care and services provided.

• Collaborate with the care team to develop and implement individualized care plans.

• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
