Work Environment This position is entirely onsite, with care delivered in the patient's home. Our team operates Monday to Friday with a standard 40-hour workweek, allowing for a consistent schedule that supports work-life balance. As part of our home health care team, you will collaborate closely with colleagues to ensure integrated care that is both holistic and patient-centered. Key Responsibilities Conduct thorough evaluations of patients referred by physicians, assessing their physical capabilities, limitations, and needs. Develop, implement, and adjust personalized treatment plans that align with physician directives and patient goals. Utilize evidence-based methodologies to administer therapeutic exercises, manual therapy techniques, and modalities aimed at improving patient outcomes. Deliver exceptional patient care by demonstrating strong communication skills with patients and their families. Document treatment progress and modifications accurately in our electronic health record (EHR) system to meet compliance standards and support interprofessional communication. Provide education and training to patients and caregivers, empowering them with knowledge and skills to aid recovery and prevent further injury. Participate in regular team meetings to discuss patient cases, share best practices, and enhance collaborative care efforts. Meet established performance metrics and utilization goals to support the overall business objectives of the organization. Patient Population / Client Focus Our patients encompass a diverse array of specialties, including orthopedic injuries, pediatric conditions, neurological disorders, and sports-related rehabilitation. The focus is on creating individualized treatment experiences that consider not only the clinical aspects of recovery but also the psychological and emotional support required during the healing journey. Qualifications A Bachelor's or Doctorate degree in Physical Therapy from an accredited APTA or CAPTE program. Current state licensure in Physical Therapy, in accordance with New Jersey regulations. Certification in Basic Life Support (BLS) from the American Heart Association. Strong written and verbal communication skills essential for effective patient interaction and documentation. Proficient computer skills, including familiarity with electronic health record systems and standard office applications. A genuine desire to build relationships with patients and a commitment to meeting their functional goals. A proactive approach in engaging with the community to foster overall health and wellness. Preferred Experience Prior experience in home health care or clinical outpatient settings is an asset. Experience working with a diverse patient population across age groups and condition types. Demonstrated ability to apply evidence-based practices in treatment strategies. Compensation and Benefits The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
