Work Environment

This full-time position is primarily based onsite, allowing for direct patient interaction in their homes. You will work within a compassionate and supportive team of healthcare providers, including physicians, nurses, and social workers, who collaborate seamlessly to ensure each patient receives individualized care tailored to their unique needs. Your role will often involve traveling to patients' residences, making it essential to have a valid driver's license and reliable transportation.

Key Responsibilities

• Conduct thorough assessments of patients in their home environment, focusing on both acute and chronic health issues.
• Develop, implement, and monitor comprehensive care plans in conjunction with healthcare team members to ensure the best outcomes.
• Order and interpret diagnostic tests as needed, making clinical decisions that optimize treatment pathways.
• Prescribe medications and therapies based on established guidelines and patient needs, carefully explaining their use to patients and families.
• Educate patients and their families on managing diseases, understanding medications, and planning for post-discharge care.

Qualifications

Compensation and Benefits
