Travel Physical Therapist (PT) – Home Health Sacramento, California Contract Duration: 13 Weeks (Mar 31, 2026 – Jun 30, 2026) Estimated Weekly Pay: $2,204.62 Schedule: Day Shift | 5x8 Hours Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a Home Health assignment with Mercy Home Health in Sacramento, CA. This role involves delivering in-home physical therapy services , focusing on patient recovery, mobility improvement, and independence in a real-world environment. Facility Information Facility: Mercy Home Health Location: Sacramento, CA Setting: Home Health Charting System: HomeCare HomeBase (HCHB) Clinical Responsibilities Perform in-home patient evaluations and assessments Develop and implement personalized treatment plans Provide gait training, mobility, and strength rehabilitation Assess home environment for safety and functional needs Educate patients and caregivers Monitor progress and communicate with physicians Maintain accurate documentation using HCHB EMR Clinical Highlights Home-based patient care (independent work) Mileage reimbursement (GSA guidelines) OASIS-based documentation Flexible and autonomous workflow Required Qualifications Active California PT License BLS Certification OASIS Experience REQUIRED Minimum 2 years healthcare experience Home Health experience preferred Additional Requirements Valid Driver's License & Auto Insurance NPI Number required Strong documentation and communication skills Scheduling & Assignment Details Schedule: 5x8 Days Hours: 40 hours/week Weekend: Every other weekend (EOW) On-call: Not specified Important Notes Must have OASIS + Home Health experience Requires 2 supervisor references at submission Mileage must be tracked and approved Cannot have worked with CommonSpirit in past 1 year Why Consider This Assignment High pay ($2.2K/week) Independent work environment Mileage reimbursement Strong demand specialty (Home Health) #J-18808-Ljbffr
