Travel RN – Hospice Care Manager

Location: Roanoke, VA

Contract Length: 13 Weeks

Schedule

• Shift: Days
• Hours: 8:30 AM – 5:00 PM
• Schedule: Monday–Friday (Flexible)
• 4 x 10-hour shifts may be considered
• No weekend requirement
• No traditional on call, but candidates should be comfortable with the standard responsibilities of a Hospice RN.
• Mileage reimbursement: $0.65 per mile

Position Overview

Good Samaritan Hospice is a highly respected, community-based, non-profit hospice organization serving the Roanoke and New River Valleys. The RN Care Manager provides compassionate, patient-centered hospice care in patients' homes and other care settings.

Responsibilities

• Manage and coordinate hospice patient care.
• Provide symptom management and crisis intervention.
• Educate patients and families regarding care plans.
• Collaborate with an interdisciplinary healthcare team.
• Travel to patients' homes, long-term care facilities, hospitals, and community agencies throughout the service area.

Minimum Requirements

• At least 1 year of recent Hospice RN experience
• Active Virginia RN License or Compact RN License
• Valid driver's license
• Reliable transportation
• Good driving record
