Work Environment

The position is onsite, delivering therapy directly in patients' homes. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes to assess their physical conditions and rehabilitation needs.
• Develop and implement individualized treatment plans, utilizing techniques such as therapeutic exercises, manual therapy, and the use of assistive devices as necessary to aid recovery.
• Administer treatments aimed at easing pain and improving mobility, adjusting plans based on patient progress and feedback.
• Document treatment sessions meticulously, recording patients' responses and progress to maintain compliance and enhance care delivery.
• Provide education and support to patients and their families on therapeutic techniques, rehabilitation goals, and assistive devices, empowering them to participate actively in their recovery.
• Stay current with best practices in physical therapy by attending educational workshops and participating in continuing education opportunities.
• Collaborate with a multidisciplinary team of healthcare professionals to coordinate care and optimize patient outcomes.

Patient Population / Client Focus

Your role will involve working with a diverse range of patients, from children recovering from sports injuries to elderly adults rehabilitating after surgery. Each patient presents unique challenges and goals, allowing for a rich variety of case experiences. You will be a significant contributor to each patient's journey towards improved health and function, helping them achieve their personal rehabilitative targets.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Must possess a valid State License for Physical Therapy.

• Proficiency in word processing and familiarity with electronic medical records are preferred.

Preferred Experience

Compensation and Benefits
