Work Environment

This position is fully onsite, providing care directly in the homes of our patients located in the Saint Petersburg area. Our physical therapists operate primarily in a home care setting, which allows for tailored rehabilitation strategies in familiar environments for patients. Key Responsibilities

• Conduct comprehensive evaluations to assess the patient's history and current condition, utilizing specialized assessment tools to form accurate diagnoses.
• Develop individualized treatment plans in collaboration with patients and their families, ensuring a clear understanding of the evaluation findings and expected outcomes.
• Implement treatment protocols effectively, adapting them as necessary based on each patient's progress and feedback.
• Maintain clear and precise documentation of all patient interactions, including evaluations, treatment plans, daily notes, and discharge summaries to provide a comprehensive view of patient care.
• Collaborate with a multidisciplinary team of healthcare professionals, ensuring adjustments to care plans when necessary and integrating additional expertise as required.
• Engage in proactive discharge planning, preparing patients for continuation of care beyond therapy sessions.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
