Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Texas Physical Therapist license required to start the assignment.
BLS certification from the American Heart Association or American Red Cross may be required.
Outpatient experience required, including evaluation and 60-minute initial evaluations with ongoing skilled treatment sessions.
Experience documenting in electronic medical records, writing measurable goals, and completing letters of medical necessity.
Ability to develop and modify individualized treatment plans and supervise discipline-specific therapy services.
Must comply with state practice act and maintain professional licensure and required continuing education.

Assignment Details
Facility Type: Outpatient clinic.
Duration: 13 week assignment.
Shift Pattern: 5x8 schedule.
Caseload: 60-minute initial evaluations plus treatment sessions as assigned.
Responsibilities: Comprehensive patient evaluations, documentation, goal writing, charge entry, and participation in student supervision as appropriate.
