Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

A clinical pharmacist role is available in New Haven, CT, offering an opportunity to apply your expertise in medication therapy management within a dynamic healthcare setting. Key Responsibilities:

Review and assess patient medication therapies for appropriateness and efficacy
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement patient care plans
Provide medication counseling and education to patients and healthcare staff
Monitor patient responses and adjust treatments as necessary
Participate in clinical rounds and contribute to the development of pharmacy protocols and policies

Desired Qualifications:

Licensed pharmacist with current credentials in the state of Connecticut
Proven experience as a clinical pharmacist or in a similar clinical setting
Strong knowledge of pharmacotherapy, drug interactions, and pharmaceutical care
Exceptional communication and interpersonal skills
Ability to interpret clinical data and apply evidence-based practices
